More likely to be for the primary effect rather than "side" effect. In many ways Ritalin (methylphenidate) is similar to cocaine in the way that it affects the brain. It actually describes an experience I've had a few times whilst falling asleep. In my case it is a very short loud roaring noise. It has never worried me so I have never bothered to pursue it. I have occasionally had something similar, starting 10-15 years ago in Switzerland, but I've not noticed it lately. Basically, some noise wakes me up -- the doorbell, the 'phone, the alarm, etc. -- but as I'm waking, the rational part of my brain says, "That sounds nothing like your [doorbell|'phone|alarm|etc.]!" and I'm usually back to sleep in short order. All my immediate family have died of strokes, so I have no personal experience of a stroke being survivable.
